Schedule Your Appointment
Book online or call us to schedule your appointment with Jason Smith, DNP, FNP-C. A link will be sent to the email address provided. Follow the links to complete the questionnaire and patient consents.
Consultation & Evaluation
During your consultation, Jason will review your information and discuss if our weight loss program is right for you. After that, the next steps will include:
1. Get blood work drawn to complete the evaluation, which requires a one-time fee that includes labs and provider evaluation. Recent labs may be used for a lesser fee.
2. Evaluation - Jason will help you map put your weight loss journey pending acceptable lab results.
Taking Medications & Beyond
Semaglutide (Ozempic, Wegovy) & Tirzepatide (Mounjaro, Zepbound) for weight loss requires that individuals take once weekly subcutaneous injections.
Phentermine is a pill taken once daily in the morning.
These medications are the most popular but other options are also available. To be successful, all medications require you to making healthy changes to your diet, exercise, and lifestyle.
Having extensive personal experiences with obesity and weight loss, Jason will help guide you on your weight loss journey.

Cost Includes:
Medication (can be administered in office or supply provided for home administration)

Monthly Visit Fee $75
This includes the monthly visit as well as dispensing of 30 day supply of medication.
As phentermine is a controlled medication, patients are required to be seen by Jason monthly. This will ensure any issues, obstacles, and plateaus are discussed and also allows for dispensing of prescription.
Fees must be paid prior to ordering labs and receiving medications. We do not submit claims to insurance. We are a cash practice, and you must pay with cash, check, or card.
We also accept HSA and FSA payments.
